Effects of different positive end-expiratory pressure titrating strategies on oxygenation and respiratory mechanics during one- lung ventilation: a randomized controlled trial

نویسندگان

چکیده

Background: Positive end-expiratory pressure (PEEP) is widely used to reduce the risk of hypoxemia and atelectasis during one-lung ventilation (OLV); however, optimal strategy for PEEP titrating remains unclear. The purpose study was investigate effects different strategies on oxygenation respiratory mechanics OLV.

Effects of Alveolar Recruitment and Positive End-Expiratory Pressure on Oxygenation during One-Lung Ventilation in the Supine Position

PURPOSE Hypoxemia during one-lung ventilation (OLV) remains a serious problem, particularly in the supine position. We investigated the effects of alveolar recruitment (AR) and positive end-expiratory pressure (PEEP) on oxygenation during OLV in the supine position. MATERIALS AND METHODS Ninety-nine patients were randomly allocated to one of the following three groups: a control group (ventil...

Reducing tidal volume and increasing positive end-expiratory pressure with constant plateau pressure during one-lung ventilation: effect on oxygenation.

BACKGROUND It is no longer safe to use large tidal volumes (V(T)) (>8 ml kg(-1)) for one-lung ventilation (OLV), and limiting plateau pressure should be a major objective. Due to the specificity of OLV, the use of positive end-expiratory pressure (PEEP) remains controversial. This study determined whether at the same low plateau pressure, reducing V(T) and increasing PEEP were not inferior to l...

The prophylactic effect of different levels of positive end-expiratory pressure on the incidence rate of atelectasis after cardiac surgery: A Randomized Controlled Trial

Background: The use of positive end-expiratory pressure (PEEP) can have an important role as one of the ways to prevent and treat atelectasis, but it seems that there is still no consensus about its beneficial level. The aim of this study was to determine the effect of different levels of PEEP on the incidence of atelectasis after heart surgery.    Methods: This is...

Effects of PEEP on oxygenation and respiratory mechanics during one-lung ventilation.

BACKGROUND One-lung ventilation-related hypoxaemia (OLV-RH) can occur in patients with healthy lungs. In this case, PEEP frequently improves oxygenation. The aim of this study was to determine, in a healthy lung model of OLV, whether the increase in PEEP improved oxygenation and whether the mechanisms involved include both inspiratory lung recruitment and an end-expiratory lung volume increase....

Effects of Positive End Expiratory Pressure On Regional Ventilation-Perfusion Matching And Respiratory Mechanics: A Clinical Study

Introduction In intubated acute respiratory failure patients, inhomogeneity of ventilation-perfusion matching (i.e., presence of over-ventilated under-perfused lung regions) might determine extreme pH shifts and increase the risk of developing ventilator-induced injury. Positive end-expiratory pressure (PEEP) redistributes tidal ventilation towards more perfused dependent lung regions, potentia...
